    Request to Observe


    ​In order for a Sequitur class or student to be observed by a clinician, doctor, or anyone interested in observing for professional purposes (excluding personal observations by current parents or prospective parents), this form must be read and agreed upon in its entirety by both the “observer” and the Headmaster. Upon reading, please complete the online request below. 
     
    Allowed Purposes for Observation
    Further academic evaluation of an individual to supplement clinical evaluations
    Further psychological or social evaluation of an individual to supplement clinical evaluations
    To write a press-release or article on education
    To learn more about classical Christian education for the purpose of supporting it
    To fulfill an academic requirement to observe a class or teacher
    To consider whether one wants to be a financial donor of the academy
     
    Conditions for Observation
    • The observer may not observe for more than three class periods in a single day. 
    • If the observer is coming observe a student, the observer must meet with the parent(s), the administration, and the teacher ahead of time to have a clear idea of the purpose(s) for observing.
    • The parent(s) must read the online request submitted by the observer and must sign they are in full agreement for allowance. 
    • Upon arrival, the observer must check in at the front desk and wear a guest pass at all times.
    • No pictures, video, or audio recordings are allowed during the observer's visit.
    • The observer will neither interview nor talk at length with any student or teacher while on campus, except for formal meetings requested by the Headmaster. 
    • No findings outside of specific observations of the student or teacher (the subject of the observation) will be legitimized.
    • Outside of specific care for the student or teacher (the subject of the observation), no research is to be conducted and nothing from this allowance to observe will be published or used in published research.
    • The observer is not to be alone with a child, their client or otherwise, at any time while on campus.
    • No personal student or teacher information is to be collected for any purpose. 
    • At any time and for whatever reason, in accordance with our Student Handbook, the observer may be asked to leave the campus. 

    ​All of this is intended to protect the integrity of the educational environment as well as the safety and integrity of the other children in our care.

    The following questions must be completed by the person coming to observe. If there are two or more people coming to observe, a separate form must be completed by all individuals.

    By placing your name in the signature box below, you have fully read and understood the conditions of this request. While on campus, you agree to fully submit to the conditions of this request. Any failure to do so will result in a cancellation of the observation agreement. Upon receiving your request, the administration will review the details of your request and contact you to determine the best path forward.
